Families Response Rate: 17.4% - 109 out of 626 families

• There are three ways to submit the survey:

Paper Survey-

• Paper surveys were backpacked home with your child. Please complete the survey and return it sealed to your child's teacher.

Online Survey-

• Families have the option to take the online survey at NYCSchoolSurvey.org in any of the 9 DOE supported languages.

• Enter the lowercase letter f followed by your child's student ID number(OSIS) and complete the survey.

• For example: f123456789

• Families can login to their https://www.schoolsaccount.nyc to complete the NYC School Survey without an access code.
